1. Services

1.1 The services shall be as described on any invoices, quotation, work authorisation or any other forms which are provided by the Dentist (Which shall mean Genuine Smiles Ltd trading as Tooth Works) to the Patient.

2. Price & Payment

2.1 The Price shall be as indicated on invoices provided by the Dentist to the Patient in respect of the services supplied.

2.2 Payment is due on day of delivery of the services, unless stated otherwise.

2.3 If the Patient is a minor then the parent/guardian/authorised person responsible for the Patient will be responsible for the payment for the services provided by the Dentist.

3. Default & Consequence of Default

3.1 If the patient owes the Dentist any money the Patient shall indemnify the Dentist from and against all costs and disbursements incurred by the Dentist in recovering the debt (including but not limited to internal administration fees, legal costs on a solicitor and own client basis, the Dentist’s collection agency costs and bank dishonour fees.)

4. Cancellations

4.1 In the event that the Patient cancels delivery of the service within 24 hours of the appointment, or fails to attend the appointment then the Patient may be liable for a fee.

5. Privacy Act 1993

5.1 The Patient authorises the Dentist or the Dentist’s agent to

a) Access, collect, retain and use any information about the Patients; (including any overdue fines balance information held by the Ministry of Justice) for the purpose of assessing the Patients creditworthiness;

b) Disclose information about the Patient, whether collected by the Dentist from the Patient directly or obtained by the Dentist from any other source, to any other credit provider or any credit reporting agency for the purposes of providing or obtaining a credit reference, debt collection or notifying a default by the Patient.

c) Disclose information about the Patient to a third party supplier for the purposes of providing additional services to the Patient (including but not limited to referrals to specialists, ACC and appointment and recall reminders).

5.2 Where the Patient is an individual the authorities under clause 5.1 are authorities or consents for the purpose of the Privacy Act 1993.

5.3 The Patient shall have the right to request the Dentist for a copy of the information about the Patient retained by the Dentist and the right to request the Dentist to correct any incorrect information about the Patient held by the Dentist. The Dentist may charge the patient an administration fee for providing copies of such information.

6.Health & Safety

6.0 The Patient agrees to undertake a blood test in the rare occurrence of accidental injury to staff during handling of used dental instruments.
